Making a referral for an Autism Diagnosis

There are a few ways to make a referral to find out if your child has autism.

The Autism Hub accepts referrals for children & young people (CYP –  up to the age of 18), registered with a GP in Bristol and South Gloucestershire, who are experiencing difficulties and differences that are associated with Autism.

PLEASE NOTE – the criteria for acceptance has recently changed. For up to date information, see the page ‘Early Health Care Needs Assessment Request’ below.

This page contains links to the forms that you will need to complete.

Parents and carers can make their own referral.

Health care professionals such as GP’s or School Nurses can also make a referral, and so can school staff such as your child’s teacher or the school SenCo.
